Where does nuclear matter come from, what causes it to give off radiation, & how does it mutate things?

I'm basically wondering where nuclear matter forms or resides, why it gives off radiation, and what does radiation do to actually mutate organisms? (I know usually you would die of radiation poisoning, but for the rare chances you mutate, what does it do to your body to cause that mutation?) I'd like this answer in simplest form.
